Welcome! We are glad that you are interested in attending Smyser School. Please read the excerpt below
regarding protocol for enrollment. If you are not sure which school your child should attend, please call 773.553.1000 or go to the CPS web site, http://schoollocator.cps.k12.il.us . Once you have verified that Smyser is your home school, please fill out as much information on the forms that are provided on this page and bring with you to the school. 
 
The parent, legal guardian, or temporary custodian of any elementary or high school age child seeking enrollment in a Chicago Public School for the first time shall present to the school principal or his/her designee proof of age, proof of current address and physical exam and immunization records. (Board Policy 04-0526-PO2)
